Swing Bed Program
A swing bed program offers transitional care for patients who no longer need acute hospital services but aren’t quite ready to return home. It provides a flexible option for continued recovery and rehabilitation.
Admission Critera
- Physician referral/certification for skilled nursing and therapy services.
- Medicare: Must have a qualifying inpatient stay within the past 30 days.
- Commerical Insurance and Medicare Advantage plans may have different coverage guidlines.
Requirements
The length of stay is frequently evaluated by the interdisciplinary care team to determine patient’s optimal discharge needs, whether it be to home, assisted living, or long-term care facility.
